Initial Appointment – Booking

Step 1 – Obtain a referral from GP or Specialist

To see our Psychiatrist or Paediatrician –

Patients must obtain a referral letter from GP (usually valid for 12 months) or from a Specialist (usually valid for 3 months).

To see our Psychologist –

Patients must obtain a Mental Health Care Plan (MHCP) from a GP, or a referral letter from a Psychiatrist.

To request for TMS Treatment Program –

Patients must visit your treating GP or Psychiatrist and ask them to complete our “TMS Referral Form”. “TMS Referral Form” is compulsory in order for patients to receive TMS treatment from Mind Oasis Clinic. You can then request a TMS initial assessment online by completing our New Patient Appointment Request Form below. Once we receive your “TMS Referral Form”, our reception team will contact you to arrange an obligation-free & bulk-billed TMS initial assessment with our TMS-accredited Psychiatrist.

Step 2 – Understand the Fee and Booking Policy

We kindly request you to read our Fee and Booking Policy before requesting for an appointment.

Deposit & Cancellation Policy

 

Private patients

  • At the time of booking, a 50% deposit is required to secure your appointment (for both Initial & Follow up appointment), it can be used towards your appointment. For telehealth consultation, a full consultation fee is required to secure your appointment as the patient will not be present in the clinic on the consultation day.
  • It is fully refundable if you cancel or reschedule your appointment with at least 2 full business days’ notice. However, please take note that all credit card surcharge is NON – REFUNDABLE.
  • 2 full business days’ notice is required if you wish to reschedule or cancel your appointment, otherwise the 50% deposit will be forfeited.

Step 3 – Complete the New Patient Appointment Request Form

Please complete the below New Patient Appointment Request Form. You will be required to upload a referral letter at the time of booking request. Our reception team will give you a call once we have received your request.

  • Some of our clinicians only accept Face-to-face patients, some only accept Telehealth patients, some accept both. We appreciate if you could state your preference clearly.
  • Please note we are a private clinic and do not offer bulk-billing service. We do not accept any workcover / CTP / medico-legal patients unfortunately, please check with your GP for other options.
