Initial Appointment – Booking & Preparation

Step 1 – New Patient Appointment Request Form

We kindly request you to read our Fee and Booking Policy below before requesting an appointment. To request an appointment, please fill in the New Patient Appointment Request Form. Our reception team will call you within 2 business days to schedule your appointment.

Deposit & Cancellation Policy

 

Private patients

  • At the time of booking, a 50% deposit is required to secure your appointment (for both Initial & Follow up appointment), it can be used towards your appointment. For telehealth consultation, a full consultation fee is required to secure your appointment as the patient will not be present in the clinic on the consultation day.
  • It is fully refundable if you cancel or reschedule your appointment with at least 2 full business days’ notice.
  • 2 full business days’ notice is required if you wish to reschedule or cancel your appointment, otherwise the 50% deposit will be forfeited.

 

Worker’s Compensation & CTP patients

  • At the time of booking, a deposit as stated below is required to secure your appointment.
    – $300 for Doctor’s initial appointment; $150 for Doctor’s Follow Up Appointment
    – $150 for Allied Health Professional’s Initial and Follow Up Appointment
  • It is fully refundable if you cancel or reschedule your appointment with at least 2 full business days’ notice.
  • 2 full business days’ notice is required if you wish to reschedule or cancel your appointment, otherwise the deposit as stated above will be forfeited.

Step 2 – Patient Portal

Once the appointment is confirmed over the phone, the system will send you an email with your Patient Portal access with 24 hours. Please activate the Patient Portal immediately after you received the email. Please check your junk mailbox as well and contact us at 9011 6800 if you have not received the email.

Patients are required to complete the below 4 steps in the Patient Portal at least 7 days prior to your initial appointment
(Both Face to Face & Telehealth)

Step 1
Fill in your details in “My Profile” (as appears on Medicare card & ID)

  

Step 2
Upload a.) Referral Letter from a GP or Specialist & b.) Relevant Medical Record to “Upload & Shared Files”

Step 3
Complete Pre-Appointment Questionnaires (for some specific clinicians)

  

Step 4
Understand and agree with our Clinic Policy
