INITIAL APPOINTMENT – PREPARATION

Step 1 – Activate the “Patient Portal” and Complete the “Patient Intake Form”

Once your appointment is confirmed, patient is required to activate the “Patient Portal” and to complete the “Patient Intake Form”. 

Once your appointment is confirmed over the phone, the system will send you an email with your Patient Portal access within 24 hours. Please activate your Patient Portal immediately after you received the email (the link will expire after 7 days). Please check your junk mailbox as well. If you did not receive any email, contact us at 9011 6800.

The Patient Intake Form will be emailed to you separately with the email subject “Mind Oasis Clinic – Patient Intake Form”. To prepare for your initial appointment, you are required to complete the Patient Intake Form (e.g. Patient Details, Consent Form, Questionnaires, Medicare Details, Payment Details, etc.) and also the Questionnaires if applicable. The link is valid for 30 days only.

  • The patient’s name must be the same as appears on the Medicare card, otherwise, patients may not get the Medicare rebate.
  • The field “Preferred Name” must be left empty, otherwise, patients may not get the Medicare rebate.
  • The Health Fund section must be empty, otherwise patients may not get the Medicare rebate. As per Australian regulations, all of the Specialist outpatient services are not covered by any health Fund.

Please note that the Patient Intake Form (and questionnaires if applicable) are compulsory and must be completed at least 14 days before your appointment, otherwise your appointment may be automatically cancelled by the clinic without any further notice.

Step 2 – Upload Documents

 

If patient needs to send us any documents prior to the appointment, patient must upload all documents via the link below:

This link is only for uploading documents, such as Referral letter, Adult Observer ADHD questionnaire, Medical records, Consent form etc. We only accept documents in PDF or JPG format. Max. file size 10 MB.

Step 3 – On the Day of the Appointment

If your appointment is a telehealth video appointment, please enter the waiting room below at your appointment time via below link:

**Please note that while the time for the appointment time is set, our Clinicians may sometime run late from their previous appointment, please be patient and wait for our clinicians. We aim to keep the scheduled appointment on time. However there may be time delays due to previous appointments and we ask you to allow a reasonable time frame (approximately 30 minutes) around your appointment time.

Please note that all telehealth appointments will be done via Video only, not via Phone.
