Initial Appointment – Preparation
Step 1 – Complete “My profile” in the Patient Portal
Once your appointment is confirmed, the patient will be given access to the Patient Portal.
Once the appointment is confirmed over the phone, the system will send you an email with your Patient Portal access within 24 hours. Please activate your Patient Portal immediately after you received the email (the link will expire after 7 days). Please check your junk mailbox as well. If you did not receive any email, contact us at 9011 6800.
- Log into the Patient Portal and complete “My Profile”.
- The paitent name must be the same as appears on Medicare card, otherwise patients may not get the medicare rebate.
- The field “Known as” must be empty, otherwise patients may not get the medicare rebate.
- The Health Fund section must be empty, otherwise patients may not get the medicare rebate. As per the Australian regulations, all of the Specialist outpatient services are not covered by any Health Fund.
Step 2 – Complete Patient Consent Form, Pharmacy Details and Pre-appointment Questionnaire
Patients must complete the Patient Consent Form, Pharmacy Details and Pre-appointment Questionnaire, and upload them in PDF format to the Patient Portal “Upload & Shared Files” section.

Patient Consent Form – Psychiatry consultation (To be completed by all Psychiatry patients)
Pharmacy Details (To be completed by patients who have a Doctor Telehealth appointment)

*** Please note that the above is compulsory and must be completed at least 7 days before your appointment. Failure to do this will result in appointment being cancelled by the clinic.
Step 3 – Upload referral letter
Patient must upload referral letter to the Patient Portal “Upload & Shared Files” section, if you have not provided a referral letter at the time of booking.
To see our Psychiatrist or Paediatrician, patient must obtain a referral letter from GP (usually valid for 12 months).
To see our Psychologist, patient must obtain a Mental Health Care Plan (MHCP) from a GP.
Workcover patients must obtain a worker’s compensation approval from the insurer.
Step 4 – Upload any medical record (if available)
Patients can upload any relevant medical record (if available) to the Patient Portal “Upload & Shared Files” section.
Relevant medical record includes:
- Relevant assessment from Psychaitrist, Psychologist or other Specialist
- Relevant pathology result
- School report (if your appointment is related to ADHD)
Step 5 – On the Day of the Appointment

We aim to keep the scheduled appointment on time. However there may be time delays due to previous appointments and we ask you to allow a reasonable time frame (approximately 30 minutes) around your appointment time.
Telehealth appointment via Zoom
If your appointment is a Telehealth Appointment via Zoom, at the time of your appointment (please arrive 5 mins earlier if possible), please visit Telehealth Waiting Room and enter your clinician’s Waiting Room.
Telehealth appointment via Phone
If your appointment is a Telehealth Appointment via Phone, your doctor will call you at around the designated time from a private number.
